GETTING STARTED
Let's start with a conversation.
Whether you are a family member, participant or referrer, you can enquire about therapy and current availability.
Enquire ↗THE PROCESS
Three simple steps.
- 01
Send an enquiry
Use the practice email for a general availability enquiry.
- 02
Discuss suitability
The team considers support needs, location, availability and proposed fees with you.
- 03
Agree the next steps
If services proceed, FGT confirms the supports, funding arrangements and service terms. An enquiry is not a confirmed booking.
NDIS FUNDING
Services linked to your plan.
FGT provides NDIS services according to the participant goals and approved supports in the participant's NDIS plan. The practice requests a copy of the plan, approved supports and budget for the agreed supports.
For NDIS-related work, fees follow the current NDIS pricing rules and limits. Proposed charges are communicated before a service is provided.
What FGT confirms with you
- Participant goals and approved supports
- Plan and budget details for agreed supports
- Whether invoices go to you or your plan manager
- Proposed fees before services begin
SERVICE AGREEMENT
Key terms before services begin.
These points summarise the current Service Agreement Standard Terms. The full agreement contains the complete conditions and applies while FGT is providing services.
How do fees and invoices work?
For NDIS-related work, fees follow the current NDIS pricing rules and limits. FGT emails an invoice immediately after a session, and the agreement states that payment is required at the time of service or beforehand. Plan-managed invoices go to the plan manager; self-managed invoices go to the participant.
What is the cancellation policy?
Please give at least two business days' notice if you cannot attend. With less notice, FGT will claim the session cost from the participant's NDIS plan. A missed appointment without notice is charged at 100% of the fee. If you or your child are sick, do not attend; text the OT or AHA directly.
How is travel charged?
Travel time and kilometres apply to appointments. Travel time, road tolls, parking fees and car running costs may be claimed from the participant's NDIS plan. FGT aims to minimise travel costs and share them fairly between participants where possible.
When are reports prepared?
FGT prepares a report about OT and therapy-assistance sessions before the end of the NDIS plan year. For children, a progress report is prepared at least every 12 months. Report-writing time is claimed from the participant's NDIS plan.
How can the agreement be changed or ended?
Talk to your OT to request a change. The standard terms require four weeks' notice if either the participant or FGT ends the agreement; that notice period does not apply if the agreement is broken.
