Services provided by Forever Growing Therapy
At Forever Growing Therapy we will help you with your goals in your home, school, community, clinic or other location we discuss and agree upon with you.
At Forever Growing Therapy we provide:
- Occupational Therapy (OT): to help people develop, recover, or maintain function in daily living skills through assessment and therapy
- Therapy Assistance (TA): support therapists by implementing supervised therapy plans, working directly with participants to carry out prescribed activities
Changing the Agreement
If you want to change this agreement, please talk to your OT at Forever Growing Therapy.
If you or Forever Growing Therapy need this agreement to end, you need to give 4 weeks’ notice. You can change or cancel your child’s services at any time. Contact support@forevergrowingtherapy.com
If this agreement is broken by you or Forever Growing Therapy, the 4 weeks’ notice will not apply.
NDIS Goals
Forever Growing Therapy is required by the NDIS to provide services according to Participant Goals and Approved Supports as stated on the Participant’s NDIS Plan.
To do this we request a copy of the Plan, Approved Supports, and Budget for the agreed supports to be provided by Forever Growing Therapy.
Following assessment or review, we will create therapeutic goals with you based on this.
Reporting
Forever Growing Therapy will write a report about your OT and TA sessions for the NDIS before the end of your plan year. We will claim for the time spent on your report from your NDIS Plan.
If you need the report earlier than planned, discuss this with your OT.
Plan Review and Plan Continuations
Forever Growing Therapy will write a report on your child’s progress at least every 12 months. This will be an Occupational Therapy (OT) Summary Report or an NDIS Plan Review Report. Your child’s OT will call you 4 months before the end of their NDIS plan to start this process.
If you are accepting an NDIS offer for a Plan Continuation of your child’s plan (Rollover) please let your OT know as that may change the type of report they write.
When we write reports on your child, we will email them to you and to anyone else you nominate.
Note: NDIS Review Reports are emailed to Parent, NDIA, Local Area Coordinator and Support Coordinator.
Cancellation Policy
If you cannot come to a session, you need to advise Forever Growing Therapy 2 business days before the session to cancel.
If you don’t notify us with 2 business days’ notice, we will claim the cost of the session to your NDIS Plan.
