PRIVACY
Privacy Policy.
Forever Growing Therapy collects personal information when it is needed to provide services. This may include health and disability needs, goals, supports, strengths, challenges and communication with other professionals.
How information is used
FGT gathers information during referrals, sessions, phone calls and emails. It may come from the participant, their child, caregivers or other supports.
The information is used to provide care, develop therapy plans, communicate with supports with consent and write reports.
This webpage summarises the practice's Privacy Policy. Contact the practice if you would like more information or have a privacy question.
YOUR INFORMATION
Collection, sharing and storage.
When may information be shared?
FGT may share information or documents with doctors, therapists, teachers and other members of the support team when the participant has agreed and the sharing is helpful for therapy. Participants provide a list of people FGT may share information with and can change that list at any time.
What happens if there is a safety concern?
If concerns arise about the participant's safety or the safety of others, FGT has a legal and ethical obligation to report factual, objective information to the appropriate authorities.
Where are service records stored?
FGT keeps service records in Halaxy, Google Enterprise and Xero. Online therapy or assessment-scoring programs may also require a participant's name, age, gender and assessment results, and each program has its own privacy policy. Ask your allied health professional which programs are used.
How is payment information handled?
FGT uses payment options including Halaxy and Xero. When a credit-card payment is made through these systems, the card number is sent to Westpac to process the payment.
Can I access my personal information?
Make the request in writing. If approved, FGT will arrange a time for you to review the information with a team member. Your allied health professional will discuss any cost of providing access. If a request is not approved, FGT will provide reasons in line with the Australian Privacy Principles.
What happens after a privacy breach?
FGT follows the Privacy Act 1988 guidelines, works with the participant to resolve the issue and contacts the participant if a breach occurs. Personal information is deleted from FGT's systems when it is no longer needed.
Questions or concerns?
Contact FGT about how personal information is collected, stored or used. The practice will listen and respond as soon as possible.
